TEAM INFORMATION SHEET ~ Get this to me ASAP. I would like to get all of your forms by 2/23/06 . . . The schedule takes two weeks for me to do. If I don’t get everyone’s information early enough, then as has happened in the past, you will only know your first match when the league starts, and I will have to continue revising them during the first two weeks.

Please make sure you fill out the part about your facility contact person, and their phone & e-mail. This is the person who handles courts for your team at your facility. Load this form with as much info as you can regarding your team and their needs and wishes. Do not send me separate e-mails about changes you want to make once you’ve submitted it. All that info must be on this form, and if you forgot it the first time, you must revise it and re-fax it to me with whatever new information you need to add. This is the only form I look at when I’m making your schedules, so if you want me to know about it, put it here.
